Most apps are vague here. Vagueness is not reassuring when the stakes are this high, so this is the whole mechanism.
What triggers it
A concerning answer on a clinically informed check-in, such as PHQ-9, or a Healld Score that falls into the Crisis band. The trigger is the answer itself, not a guess about your mood.
What replaces the score
The normal flow is set aside. You get one-tap crisis lines, your saved trusted contact, and a 60-second grounding exercise, ahead of any number, streak or prompt.
What gets recorded
Only that a crisis resource was shown, plus the trigger type and severity. Never the content of what you wrote or answered.
What it is not
Healld does not watch over you in real time and cannot dispatch help. Its crisis features hand you off to real support faster; they are not a safety net by themselves.
Privacy does not get suspended in a crisis.
The same rules apply on the hardest day as on an ordinary one. Nothing is escalated to a third party, and nothing you wrote is stored to justify the trigger.
